AI Training and Data Annotation: A Guide for Healthcare Professionals

I have been working with AI lately. High-quality output relies on human verification. While I find the rapid scaling of AI technology alarming, I am more concerned about the risks of incorrect health information reaching the public.

Besides, large language models (LLMs) like ChatGPT, Gemini, and Claude perform better when informed by professional experience.

An expert verifies its output; it’s called a human-in-the-loop approach. It helps prevent AI from going off the rails. The role is data annotation, and you create and refine the information that powers LLMs.

The Process to Become an AI Trainer

When you apply to a data annotation role, you will fill out your expertise and upload your CV. Typically, the next step is an AI-automated interview. You may not get accepted to a project right away. It typically took me a couple of weeks. There is often a task-specific test to see how well you follow directions and how well your expertise aligns with the project. Once cleared, you finally gain access to the task queue.

While the pay per task is high, account for the significant unpaid upfront time required for assessments and onboarding.

Platforms for AI training & evaluation

I use a few platforms that rely on subject matter expertise to train and refine AI systems. They accept MANY professional roles beyond medical, but I am most familiar with their clinical programs.

If you are a clinician looking for flexible work that uses your specific background, these are the companies I am currently working with:

  • Micro1: They often seek specialized input to help train frontier models.
  • Outlier AI: Operated by Scale AI, this platform connects experts (human-in-the-loop) with AI companies, providing the feedback needed to make LLMs smarter and more accurate.
  • Mercor: Frequently lists roles for experts in clinical research and medical domains to benchmark and improve AI performance.

A quick word of warning: Many of these platforms are currently facing a massive influx of applicants, meaning you may experience automated waitlists, project constraint pauses, or sudden skill freezes as contracts and project needs shift.

AI models struggle with the last mile of clinical reasoning. This includes the decision-making, regulatory understanding, and practical application that healthcare professionals handle daily. By participating in these platforms, you are helping to ground these models in evidence-based practice and safety.

Real Life Drawbacks of AI Training

Lack of feedback

The problem with working with these platforms is that much of it is automated: the interviews, the rejections, the evaluations. If you don’t get chosen or get removed from a project, there’s not much feedback. The work typically goes through three expert reviewers, then an adjudicator who makes the final decision. The adjudicator will typically provide a summary review of the changes to be made or why they were accepted.

Task Rejection

Platform architecture is often brittle. If you are removed from a project, you may lose access to all pending, unreviewed tasks, regardless of the quality of your work. The system often treats removal as a total disqualification, meaning you cannot revise or reclaim work already in the pipeline.

For example, if a task costs $200 and you have three pending tasks, removing task #1 from the project affects your ability to revise tasks #2 and #3.

The Risk of “False Positives”

Automated auditing tools often struggle to distinguish AI-generated content from human-edited expertise. You may be flagged for “AI usage” even when you follow project guidelines that permit AI-assisted drafting as a starting point.

The AI Training Pro Tip

I recommend treating AI training as a side hustle. Respect the strict guidelines, but don’t anchor your financial stability to the platform’s algorithm. Protect your peace by maintaining multiple streams of income.
